This model—adapted from the .38 Double Action Perfected Model—features both single-action and double-action operation. The trigger is centered within the trigger guard, which is now integrated into the frame. Some examples found in single-action only were modified by users after leaving the factory. The revolver is equipped with walnut extension-style grips, available with or without the Smith & Wesson monogram. Note that barrels from the 1st and 2nd Models are not interchangeable with the 3rd Model frame without gunsmithing modifications, such as milling a flat and shortening the barrel lug. The standard front sight is a Paine-style target blade, pinned into the barrel rib, though various front sight options were available. The rear sight remains mounted on the barrel latch, but windage adjustment was improved on this model via opposing screws on either side of the sight leaf. Original factory boxes are lavender in color and labeled “.22 Target Perfect” (rather than “Perfected”) on the inside lid. The end label often reads “.22 Single Target Barrel,” despite being intended for the complete firearm, not just the barrel.

Some 3rd Model revolvers appear within the 2nd Model serial number range as low as serial number 4,000. The traditional serial number range for the 3rd Model is 4617–11641, with a noted gap between 9442 and 9518. Approximately 6,949 units were produced between 1909 and 1923. Serial numbers are stamped on the front strap of the grip, with matching numbers found on the barrel, barrel latch, and the rear of the ejector plate.
